Benign yellow dot maculopathy (BYDM) is a recently recognized retinal condition characterized by distinctive yellow dots in the macular area. There is also a wide phenotypic spectrum - potentially extending beyond the vascular arcades. Recent understanding of its underlying anatomic basis means BYDM may be more prevalent than previously thought.
Multimodal imaging techniques, including blue-light fundus autofluorescence (BAF) and near-infrared autofluorescence (NIR-AF), have become key diagnostic tools in this understanding. BAF shows hyperautofluorescent dots corresponding to fundoscopic findings, while NIR-AF uniquely reveals hypoautofluorescent areas. These imaging patterns, combined with typically normal optical coherence tomography (OCT) results, allows us to differentiate BYDM from more serious macular disorders.
Accurate diagnosis matters, as BYDM is non-progressive and does not impair visual function - unlike AMD, macular dystrophies, and others. It is thought these yellow dots likely represent focal areas lacking melanin in the RPE. Proper identification can prevent an incorrect vision threatening diagnosis, thereby alleviating patient anxiety, improper management, unnecessary follow-ups, and even preventing unnecessary genetic testing.
With our understanding of BYDM, recognizing its presentation becomes increasingly important for optimal management and especially our patients' peace-of-mind.
