Corneal cross-linking (CXL) is proving to be a significant and exciting advancement in the treatment of infectious keratitis in terms of efficacy and favorable results. CXL enhances the structural integrity of corneal collagen, which in turn significantly accelerates re-epithelialization to improve healing rates vs our usual topical management.
It is being shown adjuvant CXL can lead to a reduction in corneal infiltrate size and shorter healing time. Of course this may vary based on the specific infectious source (fungal, etc), but with a safety profile showing no significant increase in complications such as corneal perforation or need for penetrating keratoplasty when compared to standard treatments we are familiar with.
Understanding the potential utilization of CXL in certain clinical scenarios matters in terms of enhancing visual outcomes in various forms of infectious keratitis - directing additional investigations and refinement of indications.
